YOUR CONTACT INFORMATION

Your contact information is not shared with anyone unless you’ve given prior authorization. Your contact information you submit is sent to jackie@fearlessphysio.com. You may be asked to enter your name, email address, phone number or other details to help you with your PT experience. Your information is used to contact you for scheduling an appointment as well as to give the provider information on how you can best be served.

HIPAA COMPLIANCE

This site is not HIPAA compliant and is not used to transmit or store healthcare information. This site is specifically designed to: provide valuable information, allow patients to schedule visits, allow patients to inquire about women’s health PT services. A double log in is required to access your healthcare information once your paperwork is submitted through the portal system. Any healthcare information will be handled directly through our HIPAA compliant services once the information is received.

Fearless Physiotherapy, LLC does not sell, trade, or otherwise transfer to outside parties your Personally Identifiable Information.

Terms and Conditions

Fearless Physiotherapy reserves the right to change this User Agreement without notice. You acknowledge and agree that it is your responsibility to review this User Agreement periodically to familiarize yourself with any modifications. Your continued use of this site after such modifications will constitute acknowledgment and agreement of the modified terms and conditions.

In order to access Fearless Physiotherapy’s resources, you may be required to provide certain information about yourself as part of the registration process. You agree that any information you provide will always be accurate, correct, and up to date.

You are responsible for maintaining the confidentiality of any login information associated with any account you use. Accordingly, you are responsible for all activities that occur under your account/s.

Accessing (or attempting to access) any of Fearless Physiotherapy’s resources by any means other than through the means provided directly to you, is strictly prohibited. You specifically agree not to access (or attempt to access) any of resources through any automated, unethical or unconventional means.

Engaging in any activity that disrupts or interferes with Fearless Physiotherapy’s resources, including the servers and/or networks to which any resources are located or connected, is strictly prohibited.

Attempting to copy, duplicate, reproduce, sell, trade, or resell resources is strictly prohibited.
